Notes - As per Aunt Thelma: Sam was taking her to visit Ada in horse-drawn cutter; the horse was frisky, cutter tipped and Annie was thrown out. She died from her injuries.
1932 Death Certificate says she jumped out of a cutter when the horse ran away and she fell and did 'something' (I can't make out word) to her head.
